Course Conflict Form

“Do I need to fill out this form?”: If you are going to be habitually late or have to leave early because of a COURSE CONFLICT, you must have filled out a “Course Conflict” form.  Once you have filled that form out, your conflict will either be APPROVED or NOT APPROVED by Dr. Wiltshire.  If your conflict is APPROVED, your habitual Tardiness is “excused” and will not affect your grade.  You DO NOT need to continually submit a “Tardy/Absent” form if you have successfully filled out a COURSE CONFLICT FORM.

Instructions:

  1. For a course override, download the Registrar’s Time Conflict Form found HERE. If you do not need the registrar to override the registration block, skip to the survey in step 6.
  2. Complete the form and enter your plan for attendance. For most of you, the plan will be to attend the non-band class and arrive late or leave early from band rehearsal.
  3. Obtain the approval of the conflicting class’ instructor.
  4. Email the form to bandadm@uoregon.edu for Dr. Wiltshire’s signature and department approval.
  5. Do NOT register for the OMB class. Register for the course in conflict with OMB on DuckWeb and the registrar will override your Athletic Band Course upon approval.
  6. Complete the OMB Course Conflict form

 

Non-Course-Related Absence or Tardy

If you DO NOT have a course conflict, but will be absent or tardy for a individual rehearsal, you MUST fill out the “Absent/Tardy” Form: (omb.uoregon.edu/absenttardy) BEFORE the rehearsal.  Each OMB member is allowed TWO (2) absences. Missing more than two rehearsals or performances will put both you and the ensemble behind and is not considered adequate course engagement. Therefore, missing more than two rehearsals will affect your grade. See the OMB Handbook for details on grading.

 If you are NOT at the rehearsal directly before a gig (ex. Thursday rehearsal before a game), you DO NOT perform at the gig and this WILL negatively affect your grade.
